Tutu Camp Waiver

I am the parent or legal guardian of the child listed above, and I have authority to sign agreements and make decisions on the child's behalf.

I hereby agree to indemnify and hold harmless Tutu School, Tutu School Franchises, all Tutu School franchised locations, all camps and programs associated with these entities, and their officers, contractors, and employees (collectively "Tutu School"), from and against any and all liability for any injury, contraction of an infectious disease or virus, or death which may be suffered by me or my child, arising out of, or in any way connected with participation in programs at Tutu School. In case of emergency, my child may be treated by a qualified physician.

I agree not to bring my child to Tutu School class or camp if either of us is sick or is suspected of carrying an infectious disease or virus. Tutu School may ask my child health questions if my child exhibits any signs or symptoms of being unwell at any time during the program.

I understand that there may be video or photographs taken of my child during their class, and I grant permission for this media to be used in faculty training and/ or promotional advertising for Tutu School or Tutu School Franchises, without compensation. I waive all rights of publicity on behalf of my child in connection with the use of this media to promote Tutu School. I understand that I may speak to a Tutu School staff member and request that my child be excluded from any photographs or videos, and that I may request digital copies of any media in which my child appears that is used by Tutu School or Tutu School Franchises for promotional purposes.

In submitting this form I agree to pay any applicable tuition or fees. In the case of class membership, I understand that I am agreeing to enroll in automatic monthly payments until I give notice to Tutu School of my desire to withdraw from the program the following month.
In submitting this form, I agree to the terms of service above. If applicable, and if I am submitting this form for a monthly class membership, I understand my card will only be authorized, not charged. If my tuition is subject to pro-ration or a discount, it will be calculated prior to my credit card actually being charged.

During the hours of Tutu Camp the only time we might be leaving Tutu School is at lunchtime, to eat and play outside (when weather permits).

I grant permission for my child to leave the Tutu School premises in the company of Tutu School faculty members, and I promise to notify Tutu School of anything Tutu School may need to be aware of (for example, a food allergy and/ or required medication) to keep my child safe away from Tutu School.

I understand that Tutu School Ballet Camp will include time outdoors. I grant permission for my child to use sunscreen at Tutu School. I grant permission for Tutu School instructors to apply sunscreen to my child if they believe my child may need it. If I send my child with their own sunscreen, I will let the Tutu School instructors know.
